


Your medical assessment must be completed within twenty-four (24) hours after you undergo the examination or, if in an inpatient hospital, as soon as possible following your release from the hospital as determined by the hospital physician. Your medical claim must be submitted in its entirety by the date listed for the examination, or as soon as possible following the examination.

You will be able to receive medical payments from GEICO for medical services performed by our professional medical providers through our MedPay service. If your medical condition results from your own negligence, you may be ineligible to receive benefits under GEICO's policies. This examination will be performed by a GEICO medical professional selected by GEICO based on your medical history, symptoms and current symptoms. Your medical examination may assist in determining whether you are eligible for the benefits described in GEICO's products. This is accomplished by a medical exam performed by GEICO's accredited provider. In order to facilitate claims evaluation and resolution, GEICO may require that you provide a medical assessment of yourself or your family member. You may change the policies described in this document by providing the appropriate notices specified to you herein. By completing this document, and by acknowledging the terms and conditions of this agreement, you consent to the use of your name and information in order to process medical claims for your care or treatment, and waive any claims for damages that are not covered under this policy provide health care services and accept payment when requested. The requirements to complete the claims process are also described in this Document. By reviewing the information contained herein, and in any event before you make a claim, you agree that GEICO will only pay for the treatment, care and services you may receive at a hospital or provider of medical services in such a manner that meets or exceeds accepted industry standards. By proceeding with these procedures, you recognize that you are agreeing to a legal commitment to resolve any dispute, claim or potential claim for your care or treatment with GEICO within the statute of limitations otherwise applicable, and you agree to promptly respond to any notification of a claim or potential claim by contacting GEICO.
